You should see your urologist again. That symptom could be a manifestation of chronic pelvic pain syndrome and chronic prostatitis can be the cause.

There can be many reasons from causes in the testicle, epididymis, vas or the prostate and requires a good clinical exam and appropriate testing by a urologist. but does not seem from a ny serious reason. Hope you feel better.
