It should not: Hiatal hernia is typically an internal hernia where a portion of the stomach is coming up through the hole where the esophagus (the tube that connects mouth to stomach) enters the abdomen. I would not think masturbation would aggravate this (unless it is very vigorous). A urologist would not be able to help with this but a gastroenterologist or general surgeon might (if it is severe).
