Work up : Work-up for a testicular mass would include history, physical, labs- afp, bhcg, ldh, chemistry, chest x-ray, and testicular ultrasound. Further treatment management depends on pathological type pure seminoma or non-seminomatous germ cell. Most testis cancer: Is confined to the testis. When spread is suspect, particularly with markers (hcg/afp), more complex imaging (ct's, head, chest, abdomen, pelvis) is done. Testis almost always spreads to abdominal & pelvic nodes before distantly to lungs and brain.
