They get smaller: A variable percentage of the breast tissue is fat, which will diminish with weight loss. That is one reason why we recommend that patients get close to a healthy weight before surgery, so we don't over-reduce. However, many women find that exercise is easier after breast reduction surgery so it can be challenging.

Loss of volume: Weight loss after breast reduction can result in some further reduction in breast volume as a portion of the breast is of fatty composition and will change with weight loss. There may be a tendency for the breasts to lose some tone and droop a little as well though most women are extremely pleased with their results. It is much easier to exercise after a reduction so some wt loss can be expected.
