Possibly: PCOS interferes with a woman's fertility, but most women with PCOS do ovulate, albeit irregularly, and can get pregnant if having sex. Even if using birth control or taking hormones, you should take a pregnancy test. Another possibility is breakthrough bleeding from an excessively thick uterine lining, a common problem with PCOS. In either case, I recommend you see a gynecologist

Not sure: 33 mm is normal for ovaries
if you have PCO you need to have 4 regular periods a year to keep healthy.
spotting may mean nothing, but do a preg test and if negative make sure your doc helps you have a period every few months
