Call 911 or go to ER: If you are having a hard time keeping yourself safe from self harm, this is the time when it is absolutely and beyond a question of doubt the time to call 911 or take yourself the nearest emergency room (if you can get yourself there safely).

Suicide hotline: Call: 1-800-273-8255. Does you psych have a separate number for emergency? Call that if so. Any friends or family who understand call and let them know how you are feeling. Go to the emergency room even if it is just to wait. Best wishes.

Working with a DR: If you are currently in therapy. Discuss these urges with your dr or therapist. Don't hide them. In many cases they are a part of treatment and may be responses to triggering events or conflicts. If you are thinking of acting on them, go to the er or call 911.
