For many - yes; for: Some- no. I am confused - procedures say behavioral therapy & the comments say 'no therapy'. Was that supposed to be 'now therapy'? Also see bilateral orchiectomy- transsexual? Remember that rome was not built in a day - the same can be said for therapy. The combination of talking therapy with psychiatric medications is definately superior to taking just one of those alone. Suicidality tx: Do you also have posttraumatic stress disorder. It is important that you be in therapy and have that addressed. Failure to do that can lead to ongoing difficulty such as you're having now. For people who have chronic thoughts of suicidality and any diagnosis in my practice i often prescribed Hydroxyzine 10 to 25 mg up to four times daily as tolerated. You can discuss that possibility with your psychiatrist. Other options include the use of lithium for its anti-suicidal properties as well as clozaril (clozapine). At the very least something should be prescribed specifically for those times when you are agitated.

meds + therapy: I would rarely advise meds alone. If you have been taking an appropriate antidepressant for months and still feel suicidal, it is time to talk to a psychotherapist.
