If you're not fasting, can doctors get a reliable result when checking for diabetes?

Yes: But it's a different procedure. It should not be over 200 if you have taken food (glucose) recently. But that is for diagnosis not for following diabetes if already diagnosed. To follow diabetes, generally the hgba1c is checked for which you don't have to be fasting - it gives your glycemic control over the preceding 3 months.

Hemoglobin A1c: Can reflect you blood sugar level in the past three months and is a good tool to have to diagnose people prone to diabetes.
