Causes: There are many causes for chest pain and a few causes for elevated troponin. Chest pain with elevated troponin is always concerning for heart attack. Depending on your symptoms, ecg, risks and level of troponin elevation, further workup may need to be done. If cardiac cath was recently done and was normal, other studies could be considered. Discuss with your cardiologist.

Unclear: I would be concerned about troponin levels going up with chest pain (how high?) but if you already had a cardiac cath, and if you were told your coronary arteries are normal that is very good news. Even without blockages, some patients have issues with cocaine, arterial spasms, clots, uncontrolled hypertension, or other conditions but only your doc can give you a diagnosis from a complete history.
