Hernias can : Incarcerate which in turn can lead to strangulation of incarcerated viscera. Best to have it repaired, especially if you have symptoms. Get an opinion from a surgeon about how long you can leave it without putting yourself in danger.

Eventually BAD stuff: It is not possible to predict when it will happen, but, given enough time, the hernia will allow contents from inside the abdomen into the sac and those contents could become stuck (incarcerated) or even lose their blood supply to become gangrenous (strangulated). If the 'stuff' happens to be intestine, you might get obstructed or need life saving surgery with possible resection & ostomy!

Unpredictable : Any untreated groin hernia can get worse, and require emergency surgery to save your life. This is however very unlikely (but unpredictable). There are two reasons to fix it: relieve symptoms and prevent it from getting worse. Best to see a general surgeon interested in hernia disease for a consultation.
