Big toe pain: Could this be deadly? Not likely. Melanoma or some other tumor could be deadly but this is very remote. It is most commonly a enchondroma (a bump on the bone under the nail). The is easily removed by your local podiatrist. You can try to change shoes and or pad the area and see if this takes pressure off the area. A x ray will often show if this is the likely cause of your pain.
