Pyelonephritis: Yes you can have relatively normal urinalysis in the presence of kidney infection (in fact saw one yesterday), but this is relatively uncommon. Kidney infection (pyelonephritis) is usually overtly symptomatic with fever, chills, back pain and most frequently abnormal urine findings.
