Ear lump: Most commonly it is just a benign lymph node. Ask your doctor about it. It would likely not have anything to do with your back.

Not related: This is not very likely related to back pain. As for what it could be? Without more information, size, appearance, timing, duration, the possibilities include a gamut of things from benign to malignant. Most commonly though a lymph node.
