Is it possible to have a heart attack with completely normal ekg? what changes are looked for besides s-t segment & t wave?

Yes: You can have a completely normal ekg and be having a heart attack. Q waves are indicative of a past heart attack but they are not always present.

Yes: It's called non st elevation myocardial infarction. Other changes are deep Q waves that indicate myocardial injury.
