Headaches: Headaches, when severe, especially in older people or diabetics, ot hypertensive, may sometimes be an early sign of stroke.

Severe headache: It sounds like this is not normal for your mom. If this is your mom's worst headache, and it is different than normal headaches, then yes go to er. Important to rule-out emergent sources of severe headaches.
