Possible: The inflammation of sle can affect the lining of many tissues. It is possible that one area is around lungs, another at the heart and another is among blood vessels. Sle patients are at higher risk for heart attacks than age matched people it would be wise to have your heart checked. Upper back / shoulder area pressure could come from irritation of liver /gallbladder area - consistent with eating.

Serositis: Usually sle serositis is resolved in two months with nsaids and steroid. I don't see you taking any of them. Have you taken new disease modifying agent? Please follow up with your rheumatologist because it looks like symptoms are lingering on you may want to have repeat cxr for pleural effusion and 2d echo for pericarditis. Thanks.

Multiorgan serositis: It's an inflammation of the outer part of organs usually manifested as pleurisy, pericarditis or peritonitis. 30-60% of sle patients manifest with pleurisy. Chest pain during deep inspiration the most common symptoms. Pericarditis could give you a pressure chest pain. Have your doctor done any heart studies like 2d echo, ecg, cxr, abdominal ultrasound to r/o gallbladder disease?
