Need work-up for ibd: You need a work-up for inflamatory bowel disease ie. Chrons or ulceratve colitis. You need a ct of your abdomen with oral contrast.

You need testing: This could be colitis, polyps, cancer, hemorrhoids. Do not risk it getting better. See a doctor for colonoscopy, cultures etc.
