Can an endoscopy pick up a hiatal hernia when a upper gi study does not?

Yes: Endoscopy and UGI study are different types of studies. It is possible for one study to pick up a hiatal hernia even when the other study did not.

Doubt it: Endoscopists see hital hernias and gastritis in every patient. Has anyone seen a clean egd?

Unlikely : An UGI interpreted by a qualified radiologist is even more sensitive at identifying a small hiatal hernia than an egd.
