Not healed: Most meniscus tears do not "heal". They can become asymptomatic. Whether the tear is actually producing symptoms depends on the tear.

Depends: A torn meniscus does not usually heal on its own. However, sometimes it will become asymptomatic (i.e. Doesn't hurt). Depending on the overall degenerative changes within your knee, pain may be from the torn meniscus tissue, or from arthritis.
