Other factors: In the heavily-affected regions of africa, lack of circumcision (still debated), social problems leading to promiscuity, lack of condoms and endemic violence against women. In the usa underclass, despair leads to IV drug abuse which spreads hiv. Even in the poorest parts of the muslim world and most of latin america, HIV infection is uncommon. This is simplistic -- there's much more.
