Cast pain: Well , if a cast is too tight pain is usually the first indicator. Certainly affected body parts changing color such as red or blue due to vascular compromise is an emergency. Be aware of this as well as numbness that could indicate nerve compression. See the physician that applied the cast for help - and don't wait .They would want to see you even if you are just concerned.

Severe pain: You would have severe pain and likely some loss of feeling. Simple way to test is press end of a toe if it blanches and then returns to normal color in 3-4 seconds you are good. Swelling can make it tight to elevation is key.
