Not sure: For sure your tongue seems irritated and seeing and ENT doc sounds like the right thing to do, and I am sure you have heard this before but quitting smoking is the best thing you can do for yourself. It causes all kinds of cancers from the mouth to the throat, esophagus and lungs and is just a terrible thing and something you have control over. It is your life man. Take control!

Red tongue : On the undersurface of the tongue there are usually many blue veins or varicosities. It is considered normal. A red, red tongue isn't and should be checked out. I know from experience, how hard it is to quit smoking, but it is so important for oral and systemic health. I don't know of any positive health benefits from smoking, just tragic results.
