Need to see the toe: Unclear from the posting if it is the mp joint or not. If it is you may have a hallux limitis. If it is indeed the toe and not the mpj, i'd consider a fracture. If by the nail consider an infection.

See below: There are several possibilities for this problem including tendinitis bursitis sesamoiditis arthritis. Do you often wear high-heeled shoes, narrowed toebox shoes and improper shoewear are some additional causes . Trying some over-the-counter anti-inflammatory medication as recommended on the bottle with food, unless medically contraindicated and some activity modification for a week or two maybe helpful . If this does not resolve your problems i would seek out an orthopedic foot and ankle specialist for further recommendations.
