Musculoskeletal: Upper back pain near the shoulder blade on one side and intensified by sneezing is likely musculoskeletal. In a late 30's male certain labor or sport mechanical muscle stress might cause a nagging pain intensified by movement. Muscle rest is important, and discomfort longer than 2-3 weeks becomes worrisome. If fever, cough, or shortness of breath then is concern for more urgent evaluation.
