Activity and sx: The most important thing to document is if you do get symptoms of palpitations and the exact time. Listing major activities is helpful because the reader will want to correlate unusually slow or fast heart rhythms with your activities. Noting that you don't have sx is helpful too because it will help ascertain if you're not feeling certain arrhythmias. It doesn't have to be detailed - just a word.
