Septal perforation : Is the medical term. It can cause a whistling sound when breathing in through your nose and can cause frequent nose bleeds. The bigger question is why do you have one. Repetitive nasal trauma, chronic nasal decongestant use, drug abuse, prior surgery, wegner's disease (autoimmune), or a cancer can cause this relatively uncommon problem. It can be repaired with little discomfort.

Depends on size...: Hi. Small holes can be asymptomatic or can whistle when you breath. Medium size holes can cause nose bleeds and crusting. Larger holes can cause symptoms of nasal airway obstruction or may even result in collapse of the nasal bridge causing a saddle nose deformity.

Crusting & bleeding: These are the most common symptoms of a perforation. Additionally repair of a perforation is much more difficult than that of a deviated septum. It may require a rhinoplasty approach to repair a perforation.
