Window jumper: First the obvious, why would you have jumped out of a second story window? Hopefully to escape a fire or intruder or the like! it is very possible permanent damage could have been done as a result of trauma from the fall. Zhave you ever been evaluated by your dr or a speicialist. If not do so asap!

Unlikely: The hydronephrosis is likely the result of chronic or acute ureteral obstruction such as a kidney stone, but a previous injury is possible only if it involved the ureter. You could have a congenital stricture or chronic infection. A urologist is best to help you with possible stenting or other surgery depending on the cause.
