Could be: Metatarsalgia which won't really show on MRI or your foot structure may be cause. See podiatrist for bio mechanical exam and possible pads or orthotics.

This is a hard : Question considering the MRI showed no pathology and we don't have your foot to examine. There are certain clinical tests to perform when looking for neuroma i.E molders click, and compressing the foot in certain ways. If, those tests are positive, one may have a neuritis without the neuroma and treatment would be similiar. Perhaps, try an orthotic.

MRI: Mri needs to be specific for the foot with slices of 0.2-0.3 mm. Most radiologist and techs have slices of 2-3 mm and so they are too big to determine small nerves or tears. You may have a plantar plate ligament tear which is commonly missed by nonspecialized radiologists. A cortisone shot will only mask pain but can lead to increase tearing of the ligament.
