Pain in the arm: It does sound like a nerve pain given the distribution. Wonder if it may be related to the surgery you had. You should see the surgeon who did the surgery. May need a nerve conduction study.

Nerve compression: You may not have had carpal tunnel syndrome but rather pronator syndrome a more proximal level of compression. With biceps pain most likely location of compression would be the ligament of struthers. Follow up with your hand surgeon.
