I have sharp pain in the pinky side of my hand, mostly centred on the knobbly bone (pisiform) i have golfers elbow and in the past i had carpal tunnel - xray and ultra sound confirmed no structural changes. could this be ralated to carpal tunnel ?

Doubtful: I don't think recurrent sharp pains are characteristic of carpal tunnel syndrome. If your recurrent pain is problematic I would consider having a nuclear bone scan to look for a fracture line or,if you haven't already done so, consult a hand surgeon for an evaluation and advice about treatment options.
