Worst case: Scenario is avoidable fractures with minimal or no trauma. Tamoxifen will help prevent this. Make sure you're getting proper calcium, vitamin d, and avoiding trauma. Check w/your doctor.

Not sure: Not sure i understand your question, but if you are asking what's the worst case scenario with osteoporosis, I have seen patients paralyzed below the level of the fracture from something as seemingly trivial as hitting another boats wake.
