Depends: Assuming your back pain is musculoskeletal in origin, you can consider pain management injections (e.g. Epidurals, etc.) along with physical therapy. However, surgery may be your best option and depends on your MRI findings and if evidence of progressive neurological deficit is present. Age is a minor factor in determining your candidacy for back surgery.

Surgery helps: You are 23 years old and have back pain that is not relieved by pain medication. You want to know if you should have back surgery. You could see a pain specialist for steroid injections and chronic following to try to relieve your pain. In the end if you don't find relief then you should consider surgery.
