Anytime: In my opinion, you don't have to suspend breastfeeding after a single dose of narcotics. Dilaudid is a powerful narcotic but 2 mg is a relatively small dose. If you did decide to interrupt breastfeeding anyway, i would pump and discard 24 hrs worth of milk after the Dilaudid dose.
