Shift to left : Shift to left (or "left shift") on a CBC means that there are more neutrophils or bands then normal. This can be a sign for bacterial infections or an early response to infection. Guessing that the 4.9 value is for rbc? Rbc count tells you how many red blood cells there are. Better to ask the doctor who has the test result for a complete answer.
