Pain, Jaundice: Pain from choledocholithiasis (common bile duct stones) will be identical to the pain from gallstones, since both cause pain by blocking the outflow of bile from the gallbladder. However, since the liver is also blocked, one will be more prone to infections in the liver and jaundice; lastly, these stones may cause pancreatitis, which can cause severe mid-abdominal pain.

Painful jaundice: Usually gallstones in the bile duct (choledocholithiasis) are associated with painful jaundice (yellow discoloration). One would feel upper abdomen pain with nausea, maybe vomiting. Blood tests would show abnormal liver tests. Ultrasound would show gallstones and likely dilated bile duct. Sometimes this condition is also associated with painful pancreatitis.
