Sounds like biliary: Dyskinesia, given that there are no stones and low gallbladder ef. Cholecystectomy will alleviate symptoms in 70% of patients with this diagnosis. However, if there is also elevated bilirubin, that is suspicious for obstruction. Bilirubin needs to be fractionated and if it is direct bilirubin which is elevated further studies are indicated (e.g. Mrcp). Fatty liver should not cause elevated bilirub.
