Monitor it: Whether it's worth seeking a subtle myelodysplastic syndrome is for you to decide. I trust the obvious (hypersplenism, lupus, pnh, hiv, B12 / folate (folic acid) / copper deficiency) ideas were ruled out. Trust you're not taking any folk "herbal" (they can be anything) or excess zinc. This might be time to adopt "watchful waiting" rather than seek a diagnosis. Best wishes.

Pancytopenia: Pancytopenia may be due to decreased bone marrow production or bone marrow failure, disorders of blood producing cells, or other conditions. There are numerous underlying conditions that could be responsible. You have been to one of the premiere centers for diagnosing this type of condition. I would follow up with your providers at sloan kettering and follow their advice - they are the experts.
