High wbc for over 3 years plus newly found high platelet count what causes this?

Need some more: Information here. With both being "high" would think of a myeloprolifierative disorder like chronic myeloid leukemia or essential thrombocythemia. On the other hand if the counts are just a bit high could be as simple as heavy smoking.
