Many things: Pain in the heel where the achilles tendon attaches can be many things. Something as simple as a retrocalcaneal bursitis from bone rubbing against your shoe to insertional tendonosis to a partial rupture or tear. This needs to be evaluated and appropriate treatment followed. A small heel lift is a good place to start and rest rom impact activity. Mri often necessary to determine the diagnosis.

Massage it: Ice it, try using a heel lift. A night splint can be efficacious. If pain persists see a foot/an ankle doctor. Gentle stretching can be helpful but sometimes may hurt while there is pain.
