BIG difference!: Antacids "neutralize" (raise the ph level) of stomach contents (including the acid already produced) whereas esomeprazole (and other proton pump inhibitors) work to inactivate acid production by the stomach. Of course, depending on the antacid you choose and/or your dose and duration of Nexium use, side effects will vary.
