Not necessarily: But it is certainly elevated and needs investigation, particularly if it's a recent increase from a lower, more appropriate value.

PSA prostate cancer: Generally a psa level above 4 warrants a follow up with your physician. There can be increases due to increasing age, size of prostate, or infection. I would definitely recommend a follow up with your doctor who will have you see a urologist for further care.

Maybe: The previous levels, rate of rise and exam findings are helpful. In a 57 years old patient i would strongly recommend a biopsy.
