PAH : Echo with pulmonary hypertension and no ti would be very unusual. There are other ways to estimate pa pressure but if no ti it probably isn't very high.

Yes: In experienced hands pa pressures can be estimated. However it is not the ultimate test of pulmonary pressures esp pah. That requires a right heart cath.
