Treat the cause: Most important is treating the source, which often remains elusive or damage irreversible, necessitating need for symptom treatment. Many types of nerve pains exist & making the distinction guides better treatment. Combo therapy is often needed augmented w/ less traditional modalities such as topicals & acupuncture. Interventional pain procedures is a candidate. Opiates work for refractory cases.

Additional Rx: Effexor, (venlafaxine) tricyclic antidepressants, ssnri's, local Lidoderm patch, a Butrans patch, all have some chance of success. However, if you possess an underlying small fibre neuropathy, or a large fibre neuropathy caused by autoimmune mechanisms, infusions of ivig may be amazingly helpful. Lastly, a medical food metanx is also of value.

Nerve Pain: It depends on the cause of your nerve pain. If there is compression or entrapment of a nerve, a steroid injection may help but surgical decompression may need to be considered. You mentioned, tn, which i assume to be trigeminal neuralgia. If you have intractable trigeminal neuralgia, you should see a neurosurgeon as there are procedures such as microvascular decompression that may be of help.
