Can you only get multiple sclerosis if you have aids?

No cause and effect.: Aids is secondary to an HIV infection, and causes suppression of the immune system, but ms is an autoimmune disorder, where one'e own body attacks areas of brain and spinal cord. One could have both disorders, but extremely rare in my experience.

Unrelated: These are two completely unrelated disorders. Patients with ms have an over-active immune system in which certain cells and the chemicals they make attack portions of the central nervous system. Aids is caused by infection with the HIV virus and results in a very under-active immune system causing the person to be very prone to infections. Where did you get this idea?
