Maybe infection: Neutrophils are the blood cells that fight bacterial infections. Lymphocytes are the blood cells that fight viral infections. If the overall white blood cell cout is elevated, and the majority of those cells are neutrophils, then there could be a bacterial infection. However, it is important to know the whole panel of numbers to accurately try to interpret.

Need more info: What is the total leukocyte count? The proportion of neutrophils and lymphocytes may be within normal range depending on other factors. Does the child have fever? All lab results need to interpreted in the clinical context and your doctor is in the best position to do that.

Why CBC?: Why was the complete blood count (CBC) test drawn on a 4.5 year old? If it was for symptoms, then the CBC likely confirms a bacterial infection. Neutrophils tend to go up with bacterial infections and lymphocytes tend to go up (or sometimes down) with viral infections.
