It indicates....: That he needs to see his doctor. The shortness of breath (sob) and back pain may or may not be related and making that distinction alone is key. A detailed medical history and physical examination obtained by your doctor, labs, ecg, chest x-ray, back x-ray may be sufficient for making the proper diagnosis. Time is key here.
