This is a hernia until proven different. He needs to be examined by a physician who will find the cause of his pain and provide a plan to get better.

This most commonly is associated with an inflammation of his epididymis (epididymitis). Of course, he needs to see his physician in order to rule out an inguinal hernia, testicular mass, torsion of his testicle (a twisting of the testicle from trauma), or lymph node enlargement.
