Tendonitis: Probably tendonitis. You need to see a orthopod, preferably a hand specialist. Treatment could be steroids or nsaid's and possibly a hand splint.

Arthritis or Tendon: This is likely to be either one of two things. Either basal joint arthritis of the thumb or dequervains tendonitis. There are other conditions but in the area you described these are the most common. Consult an experienced hand surgeon for help.
