Would add topical diclofenac gel 1%, which is an NSAID, make sure you get your rheumatologist involved as well as stronger antiinflammatory meds, possibly a short course of steroids, may need to be used.

There are a variety of option and now there are new biologic agents as Adalimumab . Etanercept . Secukinumab as well as methotrexate.
