Yes: Do you mean halfway out, which is termed partial avulsion, or do you mean halfway fractured, which means part of the crown fractures away? Both situations can occur. And both can be treated by your endodontist!

Yes: Or completely. If partial avulsion merely detached periodontal fibers, there's still hope to save it (even with nerve damage) though response to treatment (e.g. Splinting) is influenced with age, type of trauma, occlusion, habits, etc...If fracture involves the root over a mm above the junction from the crown, prognosis is poor.
